Pulse Wave Analysis: What It Is and Why It Matters
25 February 2025 · CardiAction

Every heartbeat sends a pressure wave travelling along your arteries. Pulse Wave Analysis is a non-invasive technology that reads this wave to assess the health of your arteries and the true pressure your heart, brain, and kidneys are experiencing. All of this from a standard arm cuff, with no blood test and no needles.
What Is Pulse Wave Analysis?
When your heart beats, it creates a pulse wave that travels through the arteries and reflects back from branching points in the circulation. The shape, timing, and strength of that returning wave carry detailed information about how stiff or flexible your arteries are, and about the pressure in the aorta, the large artery that supplies blood to your entire body.
Pulse Wave Analysis captures and interprets this waveform. It goes beyond the single number you get from a standard blood pressure check, giving a more complete picture of how your cardiovascular system is actually functioning.
How Does Pulse Wave Analysis Work?
The Uscom BP+ device used in CardiAction ProScreen uses a technique called suprasystolic oscillometry. A standard arm cuff is inflated above your systolic pressure, and as it slowly deflates, the device analyses the tiny pressure pulses passing through the cuff. From these waveforms it derives the central aortic pressure waveform, the pressure wave inside the main artery near your heart.
Historically, this kind of central measurement could only be obtained with invasive catheterisation, where a thin tube is threaded into the artery. Pulse Wave Analysis makes the same information available in a pharmacy, in around 20 minutes, with nothing more than a cuff on the arm.
What Does Pulse Wave Analysis Measure?
- Central systolic blood pressure, the pressure your heart and brain actually experience, which can differ substantially from an arm-cuff reading
- Augmented pressure and augmentation index, which reflect how hard your heart works against stiff arteries
- Central pulse pressure, an indicator of arterial stiffness
- Brachial systolic and diastolic blood pressure, the standard arm-cuff readings for comparison
- Heart rate, including detection of irregular heartbeat
Why Central Pressure Matters More Than an Arm Reading
The blood pressure measured on your arm is not the pressure your vital organs see. Because arteries stiffen with age, lifestyle, and disease, the pressure wave changes as it travels, and central pressure can be meaningfully higher or lower than the arm reading suggests.
Research published in Hypertension (AHA), the Journal of the American College of Cardiology, and the European Journal of Preventive Cardiology consistently shows that central blood pressure correlates more strongly with damage to organs such as the heart, brain, and kidneys, and with future cardiovascular events, than standard arm-cuff readings do.
This matters in practice. In CardiAction's own screening data, approximately 30% of people screened had clinically meaningful central findings despite a normal arm-cuff blood pressure. Two people can present with the same arm reading yet have very different central haemodynamics and arterial health. Pulse Wave Analysis is what reveals that difference.
Validated Against the Gold Standard
The accuracy question is fair, and it has a strong answer. The Uscom BP+ has been validated against the gold standard of invasive catheter measurement of central aortic blood pressure. It is believed to be the only commercial device globally to meet the International ARTERY Society Task Force's strict central blood pressure measurement accuracy criteria.

What Happens During a Pulse Wave Analysis Screening?
In a CardiAction ProScreen screening, delivered through participating pharmacies, a trained screener fits the cuff and runs the measurement protocol, which includes three readings with automatic best-reading selection. The whole visit takes around 20 minutes, with no blood test and no preparation beyond wearing something comfortable.
Your results are generated immediately and interpreted by the CardiAction platform into a personalised action plan covering what your numbers mean and the practical steps you can start taking straight away. Where findings are elevated, the platform prepares a GP letter so your doctor has the details needed for follow-up.
It is important to be clear about what Pulse Wave Analysis screening is and is not. It is an indicative risk assessment, not a diagnosis, and it works as an adjunct, not an alternative, to care from your GP. Its value is in finding meaningful signals early, while there is still time to act on them.
If you have ever had a normal blood pressure reading but wondered whether it told the whole story, Pulse Wave Analysis is designed to answer exactly that question.
